Quality Improvement Program

Trusted HP - Michigan wants to ensure quality, cost-effective health care solutions for our members. Our Quality Improvement (QI) Program aims to advance patient safety and care so our members can stay healthy and manage chronic health problems.

Trusted Health Plan’s goal is for providers to submit claims/encounters with coding that administratively captures all required HEDIS® data via claims. This will help to minimize and/or removes the need for medical record (hybrid) review.    

 Note:    The resource section of the Billing guide provides printable patient tools and educational handouts,  The Provider documentation guidelines can be printed, laminated and placed in each exam room to ensure that provider (s) in your practice  are consistent when capturing all the required data elements per measure.    

Early and Periodic Screening, Diagnostic and Treatment (EPSDT) Program

Improves the rate of members receiving health assessment screenings and testing.

Case Management Programs

Provide coordination of individualized, timely, cost-effective and integrated services to promote positive clinical outcomes for members. The purpose of Trusted HP - Michigan's Case Management program for our medically compromised population is:

  • Decrease fragmentation of health care service delivery.
  • Facilitate appropriate use of available resources.
  • Optimize member outcomes through education, care coordination and advocacy services.
